Medicaid Forms?  

I have been told by my nurses that I need to fill-out a cumbersome form for Medicare coverage of the motor (only - thanks to those rotten rascals who ruined it for the rest of us 20 years ago!!!), if I am going to buy one of these electric lift/recliner chairs.
Can anyone tell me where to get the forms (Medicare and/or Medicaid), so I can see just how cumbersome this is going to be?

To get medicare coverage for a lift chair, you will need 3 things.
• A CMN (Certificate of Medical Necessity)
• A bill of sale
• A doctor’s prescription for a lift chair and your doctor’s signature on the CMN.

Most distributors will provide the CMN straight from their website or through the mail and will also provide you with the bill of sale after purchase. There are 5 questions your doctor will need to answer on the CMN.
1. Does patient have severe arthritis of the hip or knees?
2. Does patient have a severe neuromuscular disease?
3. Is the patient completely incapable of standing up from a regular armchair or any chair in his/her home?
4. Once standing, does the patient have the ability to ambulate (walk or move about)?
5. Have all appropriate therapeutic modalities to enable the patient to transfer from a chair to a standing position (e.g., medication, physical therapy) been tried and failed? If YES, this is documented in the patient's medical records.

Your doctor will complete and sign the CMN. Once this is done by your doctor you will need to mail the CMN, bill of sale and doctor's prescription to your Medicare regional claims office. And you are also right, medicare will only cover the lifting mechanism, which is usually around $300.

I have indeed seen that most places that sell chairs will help with the forms - that's great news. I already have the prescription from my Dr.

Now, if only these places were open past 3pm-4pm in my area. The pickings are slim here and they don't even open until 11am or later. I was hoping to try a few before purchasing, since most places say that no returns are allowed. If I get a rock or sagging mess of a chair, I'll be burnt...
