Need help with lift chair  

My mother is 86 years old with 2 knee replacements, full of arthritus in her back and is a diabetic. She has a horrible time getting up from her couch or chairs. She also has to take lasix every day and she stays cold all the time. I have been looking for lift chairs and found one at Lazy Boy which has the lift, heat which should help her back and arthritus and it massages which is recommended for her diabetes. The chair is 1300.00 and I am prepared to pair the balance myself. I called Lazy Boy and they said I had to handle the paperwork myself which is fine. My problem is that this form asks for a supplier number and when I asked Lazy Boy for it she said they knew nothing about this. My question is can I get reimbursed for the lift chair from Lazy Boy or does it have to come from a company registured with Medicare.
